Welcome to the Horizon Treadmill Settlement Online Claim Form submission page.
To qualify for a settlement payment, you must complete a Claim Form submitted online or postmarked on or before April 30, 2025.
Claim Forms may be submitted by all persons in the United State of America who purchased a Horizon treadmill during the time period from June 9, 2018, through November 9, 2020, primarily for personal, family, or household purposes, and not for resale. All Settlement Class Members are eligible to claim: (i) a monetary payment, estimated to exceed $15 if there is a 10% claims rate; and either (ii) a treadmill maintenance package (valued at $50), or (iii) a tablet stand and two full months of a subscription to the mobile version of JRNY (valued at $50).
You may submit a Valid Claim if either: (a) you have received a Notice with a Unique ID number identifying you as a Class Member; or (b) you are a Class Member and can provide the serial number of your qualifying Horizon Treadmill, and possibly additional purchase information.
If you have received a Notice that includes a Unique ID and PIN, you may start your online claim by filling that information in below and clicking the "Login" button. If you have lost your Notice, please contact us by emailing info@HorizonTreadmillSettlement.com. You will be asked to provide the Class Member’s full name and mailing address.
If you did not receive a postcard Notice but believe you are a Class Member, you may start your online claim by clicking here using the serial number on your Horizon Treadmill.
